WALK-IN APPLICATION

For applicants who wish to process their documents personally at the Regional Office.

Step 1

SUBMISSION

Proceed to the Administrative Division Records Section.

Fill out the CAV Application Form.
Submit your complete documentary requirements for assessment.

Step 2

VERIFICATION

Wait for encoding.

Personnel will encode your data.
Important: You will be asked to check the draft. Kindly verify all details (e.g. spelling, birthdates, etc.) before final printing.

Step 3

PROCESSING

Finalization of Document.

Attach your ID Picture and Documentary Stamps. How to buy?
Document is printed, signed, and dry-sealed.

Step 4

CLAIMING

Receive the Sealed Envelope.

Receive the official CAV in a Sealed Envelope.
Note: Do not open the envelope. Bring it directly to the Department of Foreign Affairs (DFA) for Apostille.

APPLICATION VIA COURIER

For applicants who are currently abroad or outside the region.
